Breast Augmentation: Textured vs. Smooth (part 2 of 7)

Consultation for breast implant surgery (augmentation) is a challenging process, as there are many options to be considered. This 7-part series is intended for those of you interested in the surgery but confused by the choices involved.

Beautiful results from breast implant surgery results from excellent surgical technique, but it also relies on informed decisions by our patients. One decision you must make is Textured vs. Smooth implants. Smooth implants require smaller incisions and can be inserted through an armpit incision. They may be more prone to shifting over time, or capsular contracture. Capsular contracture is the process in which the fibrous tissue that forms around the breast implant becomes thicker and harder. A textured implant inserted partially under muscle (dual plan) will ideally adhere (like Velcro) to the muscle and chest wall. This minimizes the risk of the implant sliding down the chest wall (known as ‘bottoming out’ or ‘pseudoptosis’ of the breast), which can happen with smooth implants especially when the fold under the breast is ill-defined. If the textured implant does not adhere, perhaps from too much movement after surgery, then the implant is more likely to irritate the surrounding capsule causing thickening or fluid formation. Our preference is to use textured implants, but in patients who need to return to physical activity within two weeks of surgery, smooth implants are a safer choice.
